Application Guidelines and Requirements

 

A. REQUIREMENTS FOR APPLICATION or NOMINATION (2 copies each)

1. Application letter or recommendation letter with recommendee's conforme
2. Duly accomplished and notarized Personal Date Sheet (JBC FORM No. 1) ID picture (2 x 2)
3. Transcript of Law School Records
4. Certification of Admission to the Bar (with Bar rating)
5. Income Tax Return for the past two (2) preceding years
6. Clearances from NBI, Ombudsman, Office of the Bar Confidant and Employer
7. Clearance and Certificate of Good Standing from the IBP National Office             

       8. Birth Certificate or any other proof of age and citizenship

       9. Results of medical examination (conducted within two months prior to the filing

of the application or recommendation) and sworn
Medical Certificate with findings or impressions on the results
of medical examination.

           Required Laboratory Results:

             a. Routine Chest X-ray (x) - (Please exclude x-ray films)

             b. Hematology - CBC

             c. Routine Urinalysis

             d. Blood Chemistry - FBS, BUN, CREATININE, URIC ACID, CHOLESTEROL and SGPT

             e. Electrocardiogram (12-LD)

                      Note: You may have the above examinations done in a PRIVATE or GOVERNMENT

                               hospital/clinic as long as you submit the ORIGINAL RESULTS to us at the

                               soonest possible time.

       10. Police Clearance from place of residence
11. Treatises, opinions, or articles written, edited, or published (if any)

B. ADDITIONAL REQUIREMENTS

For Government Officials and Employees:
- Performance rating for the past three (3) years

- Sworn Statement of Assets, Liabilities and Networth for the past two (2) years

For incumbent Judge/Justice:
- Two sample decisions
 

For incumbent prosecutor:
- Two (2) sample resolutions or opinions, and the informations thereon,
if any, filed thereafter.
- Certification or verified statement as to
a. Number of complaints/inquest handled which are pending
preliminary investigation as of date of application
b. Number of complaints/inquest submitted for resolution
as of date of application
c. Number of resolutions affirmed or reversed by superiors for
the past five (5) years

For private practitioner: 

- Verified statement as to titles and case numbers of criminal or civil cases
handled for the past five years, as well as the branch numbers
and stations of courts which tried the and the status hereof.
- Sample complaint, memorandum, or appeal brief filed
with the courts in the said cases
       - Sworn Statement of Assets, Liabilities and Networth for the preceding year

       - Certificate of Compliance with, or Exemption from, the Mandatory Continuing Legal Education (MCLE) Program

For applicants for Shari'a District Courts
- Certification or verified statement as to knowledge
of Islamic Law and Jurisprudence

For applicants for the position of Ombudsman or Deputy Ombudsman
-
Affidavit stating that the applicant was not a candidate
for any elective office in the immediately preceding election.

For applicants for the position of Chairman and Regular Members (except Representative of the Law Students' Sector) of the Legal Education Board
- Certification as to the number of years in the teaching of law
in a duly authorized or recognized law school.

 

(These requirements should be submitted, within the given period, in two (2) complete sets (one original and one duplicate photocopied on a long bond paper) enclosed in separate long brown envelopes, together with (1) a transmittal letter indicating all the submitted documents; and (2) a compact disc contaning scanned or electronic files of all said documents; otherwise, the application shall not be considered.
